Community Health Resource Center Fundraiser

The Bread of Healing Clinic was founded in 2000 and provides free primary care medical services to the uninsured and under insured individuals in our community. In 2016 we were able to start a Community Health Resource Center(CHRC) that gives clients and community members access to social work services. The CHRC team has been able to complete a Social Determinants of Health screen on all patients and interested clients to connect them with community resources that address needs such as electricity, food, rental assistance, and much more. The Bread of Healing Clinic believes in treating the whole individual and we strive to provide access to resources that meet all a patient's needs. We work with other community agencies such as the Social Development Commission and Covering Wisconsin to create relationships that allow for a more comprehensive referral where we know the patient's needs will be met. A social worker or navigator is able to follow each client through their episode of need until that need has been resolved.
