Smooth Transition

The Smooth Transition mission is to empower under-served individuals with the knowledge and tools to overcome adverse circumstances and evolve into thriving members of their family, school, and community. Smooth Transitions primary outcome is for each minority at risk youth participant in our EMBRACE Program to become their own best resource through education & increased awareness

Each of our participants progress is marked in five key areas: 1. Significant increase in academic GPA by at least a full point; 2. Demonstrate awareness of the importance of financial literacy at their young age to elevate their circumstances as evidenced by tracking beginning bank account amounts vs ending bank account amounts by participating in our financial literacy projects; 3. Utilizing current resources available to them for proper health and wellness by evidence of completing our health and nutrition training; 4. Completing assigned Mentorship sessions to maintain accountability; and 5. Participate in our community based volunteer and peer mentorship programs to demonstrate their growth by giving back to their communities.
